Reactivate A Hazardous Waste EPA ID Number

Permanent State EPA ID Number Reactivation:


A permanent State ID number may be reactivated in one of the following ways:

  1. By completing and submitting the DTSC Form 1358 – Permanent State ID Number Application.
  2. Through the electronic Verification Questionnaire (eVQ) System.

Permanent Federal EPA ID Number Reactivation:


To reactivate your permanent federal EPA ID number, you may do so in one of the following ways:

  1. Through the electronic Verification Questionnaire (eVQ) System
  2. By completing and submitting the U.S. EPA Form 8700-12 – RCRA Subtitle C Site Identification Form. You may submit the form electronically or by mail. We recommend submitting the form electronically for faster processing.

Reactivate an EPA ID Number Through the Electronic Verification Questionnaire (eVQ) System

eVQ Cycle Open. Both State and federal EPA ID numbers may be reactivated when the eVQ cycle is open. To reactivate an EPA ID number in the eVQ System, please select one of the following sets of instructions:


I Have An eVQ Account


I Do Not Have An eVQ Account


eVQ Cycle Closed. If the eVQ cycle has closed, please submit the DTSC Form 1358 to reactivate a permanent State EPA ID number or the EPA Form 8700-12 to reactivate a federal EPA ID number.


Temporary EPA ID Number Reactivation

Temporary EPA ID numbers cannot be reactivated. If your temporary State or federal EPA ID number has expired or will be expiring and you still need to use an EPA ID number, depending on the type of waste and the amounts you are generating, you will apply for either a permanent State EPA ID number or a permanent federal EPA ID number.
